Dentaline Tandartspraktijk handles your personal data with care. This statement explains which data we process, why, and what rights you have.
Who are we?
Dentaline Tandartspraktijk, located at Terra Cottaplein 4, 6216 BH Maastricht, is responsible for the processing of personal data as described in this privacy statement. You can reach us via info@dentaline.nl or 043 – 234 07 09.
Which data do we process?
When you fill in the contact or registration form on our website, we process the data you provide to us. This may include: salutation, first and last name, date of birth, citizen service number (BSN), address details, phone number, email address and the content of your message. When registering family members, we also process the data you provide about these family members.
As a healthcare provider we are legally required to use your BSN when creating your patient record (Dutch Act on additional provisions for the processing of personal data in healthcare).
What do we use your data for?
- Processing your registration as a patient and creating your patient record.
- Contacting you to schedule a first appointment.
- Answering questions you ask via the contact form.
- Sending a confirmation of your registration by email.
Sharing with third parties
We do not sell your data to third parties and only share it when necessary for your treatment (for example with your health insurer or invoicing partner Infomedics) or when we are legally required to do so.
Retention and security
Form submissions are delivered to our practice by secure email and are not stored on the website. Data that becomes part of your patient record is kept in accordance with the statutory retention period of the WGBO (twenty years). Other data is not kept longer than necessary.
Your rights
You have the right to access, correct or delete your personal data (insofar as the law allows). You can also object to the processing or withdraw consent you have previously given. To do so, contact us via info@dentaline.nl. If you believe we are not handling your data correctly, you can file a complaint with the Dutch Data Protection Authority (Autoriteit Persoonsgegevens).
